Jointly provided with Regional HealthPlus in Spartanburg, SC for RHP Physicians and advanced practice providers (NP, PA), our activity was designed to improve provider quality management in achieving hypertension, lipid and diabetes control rates in patients for participating physicians in the Spartanburg Regional HealthPlus.

• Based on 2018 analyzed provider performance data, suboptimal control rates existed in the RHP Network for hypertension, diabetes, and statin use in diabetic and cardiovascular disease (CVD) patients, as compared to the evidenced-based 4 Star Health Care Effectiveness Data and Information Set (HEDIS) control rates.
• This Performance Improvement CME activity consisted of comparison of baseline to sequential performance assessments, use of on-demand education modules, face-to-face accountability check-ins, and review of performance data reports comparing performance changes to baseline.
