Affecting millions of lives across the Southeast and nationally, COSEHQ engages practicing clinicians in the evidence-based management of cardiovascular and chronic diseases, using real-time data from the front lines to identify care gaps.
COSEHQ’s Centers of Excellence are networked diverse Southeastern community practices engaged in continuous data-driven quality improvement. The Centers’ quality performance period from 1/1/2020-6/30/2021 represents over 2,500 Southeastern community-based physicians and 371,122 active patients of which 50% are diagnosed with hypertension and 18% with diabetes. Seventy percent of the patients diagnosed with hypertension met a blood pressure benchmark of <140/90mm Hg, compared to a national average of 43.7% and 77% of patients diagnosed with diabetes had an HbA1c <8%. The Centers continue to improve cardiovascular health with a focused movement towards the 2017 ACC/AHA Hypertension Guidelines blood pressure target goal of 130/80 mmHg.
